• Affiliation Agreement
• 4-H Group Tax Exception Authorization
• Monthly Finance Reports (All 12 months of the year)
• Financial Summary/ Audit Report (Treasurer's Report - Pages 20-23)
• IRS 990 Filing - Discuss and turn in all documentation with Kaena Prince to complete.
• *Change all 4-H information to county email and Extension address if you have not done so already.*

Instructions:

1. Download the 2025 4-H Achievement Bingo, and print however many you need.

2. Complete activities on the card until you get BINGO (five in a row—across, down, or diagonal).

3. Once you have BINGO, turn in your completed card along with a short document describing how you completed each square.

4. Then participants can apply this information to their Achievement Plan and Project Record Books.
